Exploring Life & Business with Jeff Crume of Golden Age Transitions

Today we’d like to introduce you to Jeff Crume.

Hi Jeff, thanks for joining us today. We’d love for you to start by introducing yourself.
I’ve spent over two decades standing in the sacred spaces of people’s lives — as a hospice chaplain, grief counselor, and spiritual care provider. My work was never about fixing people; it was about being present with them as they faced some of the hardest moments imaginable. That shaped me more than any title or job description ever could.

Over time, I felt something shifting in me — a quiet pull toward a bigger expression of the same heart. I realized I wasn’t meant to leave behind the compassion I’d built my life on; I was meant to carry it into a new arena. That’s how Golden Age Transitions was born.

Southern California has played a big part in my journey — both personally and professionally — and it’s where much of this work first took root.

Today, I help seniors and their families navigate one of the most emotional transitions of their lives: finding the right place to call home when living alone no longer feels safe. It’s deeply personal work for me. Every placement, every conversation, every family I meet reminds me that my story has always been about service — it’s just evolved into a form that lets me combine my compassion, my leadership, and my calling.

I haven’t arrived here through a straight line. It’s been reinvention, growth, loss, remembering, and standing back up again. But the through-line has always been the same: help people feel seen, supported, and safe. And that’s the heart of everything I do today.

Would you say it’s been a smooth road, and if not what are some of the biggest challenges you’ve faced along the way?
Smooth? Not even close.

My path has been made up of reinventions, detours, heartbreaks, and rebuilding seasons. I’ve walked through career transitions, personal challenges, and moments where I questioned everything — my calling, my direction, and even my confidence.

Leaving a long career in hospice to start something new was both exciting and terrifying. I went from a defined role to creating my own lane, and that came with financial pressure, uncertainty, and a lot of “Is this really going to work?” nights. Building Golden Age Transitions meant learning the business side while holding onto the heart of the work. That balance wasn’t always easy.

But the hardest part was internal. Learning to trust myself again. Learning to believe I could build something meaningful outside the structures I’d relied on for years. Learning that growth often looks like stillness, setbacks, and seasons where nothing seems to move — until suddenly everything does.

Those challenges shaped me. They clarified my purpose. And they taught me that the road doesn’t have to be smooth to be sacred. Every struggle has become part of the foundation I stand on today.

Thanks for sharing that. So, maybe next you can tell us a bit more about your business?
Golden Age Transitions is a senior placement and family support service that helps older adults find the right assisted living or memory care community when living alone no longer feels safe. In simple terms, I guide families through the entire process — from assessing care needs to coordinating tours, handling paperwork, and making sure their loved one moves into a community that feels safe, supported, and aligned with their needs.

Unlike the large national referral services where families may speak with multiple representatives and get filtered into automated recommendations, my approach is completely personal and concierge-level. Every family works directly with me, one-on-one, from the first call until their loved one is safely placed.

I’m known for bringing over two decades of hospice, spiritual care, and grief support into this work. Families don’t just get a consultant; they get someone who understands the emotional, medical, and human layers of this moment. When a family calls, they’re often overwhelmed or unsure where to begin. My job is to simplify the process, advocate for them, and help them breathe again.

I handle everything from evaluating care needs and recommending communities to navigating costs, coordinating the move, and supporting families long after placement. But what I’m most proud of is the heart behind the brand: I show up when families feel alone. I listen. I stay present. And I walk with them until they feel secure again.

Though my work began in Southern California, Golden Age Transitions has recently expanded to serve families in Mohave County, Arizona as well.

Golden Age Transitions was built on a simple belief — that no senior or family should ever have to walk this journey alone. We exist to bring clarity, dignity, and compassion to a moment that can feel overwhelming. And that’s the heart of our work, every single day.

Before we go, is there anything else you can share with us?
If there’s one thing I’d want readers to know, it’s this: senior care doesn’t have to be overwhelming. You don’t have to wait for a crisis or try to figure it out on your own. There are people — like me — who will sit with you, listen to your story, and help you take the next step with clarity and confidence.

Our services are completely free to seniors and their families. When a family chooses a community we’ve recommended, the community compensates me for helping guide them through the process. That allows families to get one-on-one support without any financial pressure.

This work is personal to me. It’s an honor every time a family lets me into their world and trusts me with someone they love. Whether it’s the first time you’re hearing the term “senior placement” or you’ve been searching for answers for months, I want families to know they’re not alone, and there is a path forward.

Golden Age Transitions isn’t just a service — it’s support, guidance, and heart at a moment when it matters most.
